Oberlin Church - Repository for forms

These are some of the more popular forms to be used by church members and regular attenders to get action taken at Oberlin. After you complete the form, save it to your computer and email it to: oberlinchurch@msn.com .

1. Purchase Request/Authorization Form - Use this form to request permission to purchase something. It must be eventually signed by the person whose budget it will come from.

2. Work Order Form - Submit to request facility repair or suggest an addition or enhancement to the building or surrounding grounds.

3. Fundraiser Request Form - used to deconflict events at Oberlin. Must be eventually approved by the Council.

4. Facility Use/Rental Form - Used by members and non-members to reserve and rent Oberlin rooms for special events. Even though there is no charge for church events, this helps us set up properly and reviews safety rules.

5. Small Group Report Form - Used by Small Group Facilitators to give updates on events in small group setting.

6. Child care worker job application - Fill out and either email or bring by the office.

7.Child care feedback form - Fill out and either email or bring by the office.

8. Child care payment authorization form - Fill out and bring to office for payment.

9. Accident Report Form - use if there is any incident or accident at the church.

10. Child care registration form - used to give our workers the necessary information about your child for our child care program.

11. Child Care Request Form - used to ask for child care for church sponsored event.

12. Washington State Patrol Background Check - submitted for anyone who works with our children.
